If you're looking for information about one of our programs, we encourage you to visit the program page before completing your registration.

Required Annual Forms

These forms only need to be completed once each calendar year unless your information changes.

2026 Waiver & Release

All riders, participants, volunteers, and visitors entering horse areas must have a signed waiver on file for the current calendar year before participating or entering those areas. A parent or legal guardian must complete the waiver for each minor.

2026 Emergency Contact Form

All riders, participants, and volunteers must have a completed emergency contact form on file for the current calendar year before participating. A parent or legal guardian must complete the form for each minor. Update the form whenever contact details or relevant medical information changes.
